You pay once a year. There are no fees, but there is an excellent dashboard that makes it very easy to move your domain every time you change hosting companies.
It doesn’t matter whether you want to put the domain on a blog like blogger, or on a hosting site.
Your price will go down if you register the domain for longer than 1 year.

I have 19 domains hosted at Godaddy.com. The price that you are quoting is most likely for a two year registration. When you attempt to register a domain name with Godaddy, they default the registration to two years (a nasty trick to up their average revenue per transaction) and then if you want to only register for one year you need to select one year during the checkout process.
Monthly fees are for hosting. Domain registration at Godaddy.com does not come with free hosting. It does come with free “parking”, which means that Godaddy.com will put their own page up on your web address. Not great if you want to actually use the address.
To use the address, you then either need to pay for a hosting package (you can pay month to month, a bit more expensive, or in advance… which is when the discounting really comes into play). I purchased 2 years in advance and got some sort of holiday special deal for spending more than $120. My total cost ended up being roughly $5 per month for hosting over two years. For this amount, I am able to host as many domains as I want – so all 19 domain names that I have at Godaddy have their own website for the grand total of $5 per month – a pretty good deal.
If you do not want to host with Godaddy, you can “point” your domain name to an IP address somewhere else on the Internet where your website is hosted. You do this through DNS management. This pointing process is very easily achieved through the account management interface within the Godaddy.com website. And if you have any trouble, Godaddy technical support is very helpful – just be aware that it is a local call and not an 800 number (last time I checked).

Price quoted by GoDaddy.com is annual fee — so there’s no monthly fees or any other fees UNLESS you sign up for other services (web hosting, etc).
If it is just purely domain name registration, then you only pay that once a year — or however long you want to pay for the domain name (I think with GoDaddy you can pay up to 10 years)

If it says $19.99 a year then thats all for a year, however when you buy it they offer you other services as long as you dont click on the then you should be fine.
